A public form driven by a Google Sheet, with emailed summaries
We build a public web form whose fields and dropdown options come from a Google Sheet you control. When someone submits it, you get a tidy summary by email. Change the sheet and the form changes, with no developer needed.
The problem
Intake forms go stale. A new service, location, or option gets added and someone has to edit the form, or the dropdowns drift out of date. Submissions then land in an inbox as raw, hard-to-scan text. If you track options in a spreadsheet anyway, keeping a separate form in sync is extra work and easy to forget.
What we’d build
- A public form page that reads its field list and dropdown options from a Google Sheet you own
- Field types set in the sheet: text, dropdown, checkbox, date, and required or optional
- A clean emailed summary for each submission, with labeled answers in the order you choose
- Saved submissions in a simple list view, so you can look back at past entries
- Hosted link and full source code, with two rounds of tweaks
A concrete example
A small property management company keeps a Sheet with one tab listing form fields (label, type, required) and another listing buildings and issue types. Their maintenance request form reads both tabs. A tenant picks a building and issue type, describes the problem, and submits. The office gets an email with the building, issue, and description at the top, and the entry appears in a saved list. When they add a building to the sheet, it shows up in the dropdown. That is about four screens: the form, a confirmation page, the submissions list, and a detail view.

Frequently asked
Does this fit within the Tool tier?
Yes. Tool includes one integration, and here that is email. The form reads your Sheet through a read-only published link. Writing submissions back to the Sheet, or adding a second service, would go beyond this scope, and we would tell you before starting.
What happens when I edit the Sheet?
The form picks up changes to field labels, types, and dropdown options from the Sheet. Changing the structure of the sheet itself, such as its columns, or adding new field types, would be a tweak request. Two rounds of tweaks are included.
What about hosting and refunds?
Hosting is included for 12 months, then $9.00 per month, or you can self-host using the source code we deliver. We give a full refund if we decline the project before work starts. Once we deliver, there are no refunds.
