An embeddable feedback form with a weekly email digest
We build a short feedback form you can embed on your site. Every week, it emails you a digest of the responses so you don't have to check a dashboard.
The problem
Most sites either have no way to leave feedback, or they have a form whose messages pile up in an inbox and get lost. Nobody reads them as a group, so patterns go unnoticed. You end up with scattered comments instead of a clear picture of what visitors are telling you.
What we’d build
- A short feedback form (a rating, a comment box, and an optional email field) that embeds on your site with a snippet you paste in.
- Saved responses, stored so each one is kept with the date and the page it came from.
- A weekly digest email that lists the week's responses, shows the count and average rating, and goes to the addresses you choose.
- A private responses page, protected by a shared access key rather than user accounts, where you can browse and search past feedback.
- A small settings screen for the digest day, time, and recipient list, plus a thank-you screen for visitors. That's five screens in total.
- Hosted link, full source code, and two rounds of tweaks after delivery.
A concrete example
A bakery with an online ordering page wants to hear how orders went. We build a form with a one-to-five rating and a comment box, and give them a snippet to paste into their order confirmation page. Responses are saved with the date. Every Monday morning, the owner and the shop manager get one email with the week's comments, the number of responses, and the average rating. The owner can also open the private responses page to look back at older feedback. After delivery, they use their two rounds of tweaks to reword the form's questions and move the digest to Sunday evening.

Frequently asked
How much does this cost and how long does it take?
It is our Tool tier: $749, delivered in 5 business days after work starts.
Can the digest also go to Slack or a Google Sheet?
Not in this build. The Tool tier includes one integration, and the weekly email uses it. Adding another connection would be outside this scope.
Do my team members get their own logins?
No. The responses page uses a shared access key, and the digest arrives by email. Separate logins and roles belong to our Mini-app tier.
What happens after the first year of hosting?
Hosting is included for 12 months. After that it is $9.00 per month, or you can take the source code and self-host.
What if I change my mind?
If we decline your project before work starts, you get a full refund. Once we have delivered, there is no refund, but you still have two rounds of tweaks to get the details right.
