A one-page intake form that emails you a PDF

We build a single intake form that turns each submission into a PDF and emails it to you, with an optional copy to the client. It is a fixed-scope Fix: $249, delivered in three business days.

The problem

Intake details often arrive as scattered emails, phone notes, or responses from a form tool that you have to open and reformat. When you need a clean record to file, attach to a job, or send back to the client, you end up copying answers into a document by hand. That is slow and easy to get wrong.

What we’d build

  • One intake form on a single page, with the fields you specify (text, dropdowns, checkboxes, dates, and similar basic inputs).
  • A PDF generated from the answers on submit, with your name or logo and a simple, readable layout.
  • An email to you with the PDF attached, sent as soon as the form is submitted.
  • An optional field or checkbox so the client can receive a copy at the address they enter.
  • A confirmation screen after submit, which is the second of our two screens. There is no login and no database of saved answers.
  • A hosted link and full source code, plus one round of tweaks after delivery.

A concrete example

A bookkeeper wants new clients to fill out one form before the first call. It asks for business name, contact details, services needed, fiscal year end, accounting software in use, and a notes box. On submit, the form builds a one-page PDF with her logo at the top and the answers listed under clear headings. She gets the PDF by email right away, and clients who tick an 'Email me a copy' box get it too. After delivery, she sends us one round of tweaks, such as reordering fields and changing the wording on the confirmation screen.

Do you store the submitted answers?

No, the Fix tier has no saved data, so answers go into the PDF and the email only. If you want a searchable record of submissions or a simple dashboard, that is the Tool tier at $749.

Can the form collect health information or card numbers?

No, we do not build anything that handles regulated data such as health records or card numbers, so those fields should stay off the form.

What about hosting and refunds?

Hosting is included for 12 months, then it is $9.00 per month, or you can self-host using the source code we hand over. We give a full refund if we decline the project before work starts, and no refund after delivery.
